Healthcare is US healthcare at US prices — for non-Americans this is the single most important insurance destination in the Caribbean, and a policy with at least US million medical cover is sensible since an ER visit bills in the thousands. US visitors should check whether their domestic health plan and network extend to Puerto Rico (many do, as it is domestic). Major hospitals cluster in the San Juan metro area (the Centro Médico complex in Río Piedras handles trauma); island-wide care thins out fast outside the metro.

STI testing runs through federally funded community health centers on a sliding scale, private labs (walk-in, fee-based), and municipal clinics; PrEP and PEP are available through the same US systems as the mainland — for PEP go to any hospital emergency room within 72 hours. Pharmacies (Walgreens, CVS) are everywhere in the metro and stock everything you would find stateside.
